A Journey Through Time: The History and Global Rise of Cosmetic Gynecology

Cosmetic gynecology might seem like a modern trend, but its roots stretch back further than you might think. For centuries, cultures around the world have explored ways to enhance or restore the female body, often for aesthetic, cultural, or functional reasons. From ancient practices in Egypt and India—where genital modifications were tied to rituals or beauty standards—to the more medical approaches of the 20th century, the desire to address women’s intimate health and appearance has always existed. But it wasn’t until recent decades that cosmetic gynecology emerged as the dynamic, specialized field we know today.

The modern story begins in the late 20th century, when pioneering doctors started blending gynecology with cosmetic surgery techniques. In the United States, visionaries like Dr. David Matlock introduced terms like “vaginal rejuvenation” in the 1990s, sparking curiosity and debate. Around the same time, Dr. Marco Pelosi III and his father, Dr. Marco Pelosi II, coined “cosmetogynecology,” laying the groundwork for a new frontier in women’s healthcare. Dr. Red Alinsod coined the procedures and phrases such as “Hybrid” and “Barbie Look” Labiaplasty and ThermiVa. What started as a niche idea—focused on procedures like labiaplasty or vaginal tightening—quickly gained traction as women sought solutions for both aesthetics and comfort after childbirth, aging, or simply personal preference.

Fast forward to today, and cosmetic gynecology has exploded into a global phenomenon. From North America to Europe, Asia, and beyond, the field has grown thanks to advancements in technology—like lasers and radiofrequency treatments—and a growing openness about women’s health. In India, for example, demand for these procedures jumped from just 3.9% in 2012 to nearly 29% by 2015, reflecting a cultural shift toward empowerment and self-care. In Europe, non-surgical options like fillers for vulvar rejuvenation are all the rage, while Brazil and Argentina have become hubs for cutting-edge techniques. This worldwide boom isn’t just about beauty—it’s about confidence, function, and giving women choices.

The International Society of Cosmetogynecology (ISCG), founded in 2004 by Dr. Marco Pelosi III and others, has been a driving force in this evolution. It’s where experts from every corner of the globe—like those I just met at our annual meeting in Fort Lauderdale—come together to share breakthroughs, refine techniques, and ensure these procedures are safe and effective. What began as a small group of innovators has grown into a vibrant community, mirroring the field’s own expansion. Today, cosmetic gynecology isn’t just a medical specialty—it’s a movement, empowering women to feel their best, inside and out.

As I reflect on our recent gathering, with Dr. Pelosi III’s inspiring “State of the Art” message and the memories captured in my photos, I’m reminded of how far we’ve come—and how much more there is to explore. For a decade we both ran Congresses on different Coasts. I ran CAVS (Congress on Aesthetic Vulvovaginal Surgery) from the California. Marco ran ISCG (International Society of Cosmetogynecology) from New Jersey. We ran our first combined Congress in 2016. under the ISCG banner to unite the Americas and we have never turned back.

Tina Batalha, MD, is one of Brazil’s rising star in Cosmetic Gynecology. She has a solid presence on Social Media and has dedicated followers of her unique surgical skills. A legend in the making.

Jennifer Hayes, MD, of Clearwater, FL, is smiling because she just retired! Congrats Jen! What an amazing career!

Neeraj and I go back about 15 year when we managed the worst pelvic prolapse and leaky bladders. He trained with me in Cosmetic Gynecology a decade ago and is now the Top surgeon in this filed in Boston.
John Miklos, MD, is a legend. Known for doing the toughest cases everyone runs away from. He presented his laparoscopic method for creating vaginas when there as none or was minimal.

Robert Moore, MD, is the partner of John Miklos, practicing in the Atlanta area. Rob gave a tour de force talk on the radiofrequency treatment of lichen sclerosis. in combination with PRP and Exosomes.
